(a) It shall be the duty of the department to insure that every applicant for, or recipient of, assistance be notified not less frequently than semiannually as to the provisions of eligibility and his responsibility for reporting information concerning changes in circumstances which may affect the amount of grant. After such notification has been provided, the department shall require the recipient to formally acknowledge, on a form prescribed for such purpose, that the provisions of eligibility and reporting obligations have been explained to him and were understood.
(b) Each applicant for or recipient or payee of such assistance shall be responsible for reporting accurately and within a reasonable specified period those facts required of him pursuant to the explanation provided by the department.
