Texas Human Resources Code - HUM RES § 40.0512. Continuity of Services;  Information Sharing

The department shall make a good faith effort to share relevant and appropriate information with health and human services agencies regarding persons receiving services from the department to ensure continuity of care and the best possible coordination of state-funded resources among health and human services agencies.
