A home collection kit for HIV infection testing shall be sold as part of a package of services that includes:
(1) laboratory testing by a qualified facility;
(2) reporting of test results;
(3) verification of positive test results;
(4) counseling as required by this subchapter; and
(5) information, upon request, describing how test results and related information are stored by the service provider, how long the information is retained, and under what circumstances the information may be communicated to other persons.
