On or before the first day of February, in each year, the operator or superintendent of every mine, bank or colliery, shall send to the inspector of the district, a correct report specifying with respect to the year ending December 31, previously, the name of the operator and officials of the mine with their post office addresses; the quantity of coal mined; the amount of powder or other explosives consumed; the number of persons employed above and below ground in or about such colliery or bank, classifying the persons so employed. The report shall be in such form as may be from time to time prescribed by the Department of Mines and Mineral Industries. 1 Blank forms for said report shall be furnished by the Commonwealth.
