On or before the fifteenth day of every month, the operator or superintendent of every mine, bank or colliery shall, with respect to the previous month, send by mail to the inspector of the district, on a form to be prescribed and furnished by the Department of Mines and Mineral Industries, 1 a report of the quantity of coal mined or produced and the man hours worked. Operators of preparation plants shall furnish the mine inspector with a monthly and annual report showing the source and quantity of coal purchased for preparation.
