A policy issued to a group, other than one described in clauses (1) through (5.1) of 40 P.S. § 532.1, and which meets the requirements of clause (5.2) of 40 P.S. § 532.1, shall also meet the requirements of this section. The premium for the policy shall be paid by the policyholder either wholly from the policyholder's funds or funds contributed by the policyholder, or wholly from funds contributed by the insured persons, or both. A policy on which no part of the premium is to be derived from funds contributed by the insured persons must insure all eligible persons, or all except any as to whom evidence of individual insurability is not satisfactory to the insurer.
