(a) Each person desiring to apply for examination shall file with the commission a formal application in which the applicant shall state under oath or affirmation (i) his full name and residence or post office address, (ii) his citizenship, place and date of birth, (iii) his condition of health and physical capacity for public service, (iv) his business or employment and his residence for the past five years, and (v) such other information as may be required by the commission's rules and regulations, showing the applicant's qualifications for the position for which he is being examined.
(b) No person shall be eligible to apply for examination unless he is at least eighteen years of age on the date of application and has been a resident of the county for at least one year immediately preceding his application.
