The written approval of the Department is effective until the Department cancels its written approval. The Department may cancel its written approval if the carrier to whom the approval is issued:
1. Fails to maintain adequate insurance;
2. Does not notify the Department of any changes in the location of the carrier’s terminal facilities; or
3. Does not comply with any applicable requirements relating to safety or financial responsibility.
