The Attorney General shall prescribe a form of addendum to a contingent fee contract that must be used for every such contract. The addendum must set forth the specific rights and obligations of the parties relating to the matter that is the subject of the contract, including, without limitation, the requirements of NRS 228.1113 and 228.1115.
