The petition shall be in substantially the following form:
“To the municipal clerk of (name of municipality):
“You are hereby requested to lay before the municipal council of the municipality of (name of municipality) forthwith the demand of the undersigned to request the county clerk to place upon the general ballot to be used at the next general election to be held in said municipality, the following public question:
“ ‘Shall the municipality (name of municipality) abandon its organization under the municipal manager form of government under subtitle 5 of the title Municipalities and Counties of the Revised Statutes (§ 40:79-1 et seq.), and resume the form of government under the law under which it was being governed when the municipal manager form of government was adopted?’ ”
