(1) When a licensed renderer or the renderer's agent receives a dead or fallen animal, the renderer or agent shall present a dead or fallen animal record to the person, corporation, or association that has requested the renderer or agent to remove the dead or fallen animal. The record must be on forms prescribed by the department and contain information the department may, by rule, require.
(2) The report must be executed in quadruplicate. The original copy must accompany the carcass and hide until the hide is officially inspected for marks and brands. The duplicate must be retained by the licensed renderer for the time that the department in its discretion requires. The triplicate must be filed within 7 days after its execution and without cost in the office of the county clerk and recorder of the county in which the animal is received by the licensed renderer or the renderer's agent. The quadruplicate must be retained by the person, corporation, or association that requested removal of the dead or fallen animal.
