Michigan Compiled Laws, Chapter 380. School Code of 1976 § 380.1723

Sec. 1723. The ballot submitting the question of the adoption of sections 1722 to 1729  1 to the school electors of an intermediate school district shall be substantially in the following form:

“Shall the _______________ (legal name of the intermediate school district), state of Michigan, come under sections 1722 to 1729 of the revised school code, which are designed to encourage the education of students with a disability, if the annual property tax levied for administration is limited to _____ mills?
