The court disposition level of the system shall consist of at least the following information as relates to counsel:
(1) Did the defendant have counsel;
(2) Did the defendant retain private counsel;
(3) Was counsel provided by the public advocate;
(4) If counsel was provided by the public advocate:
(a) Was reimbursement sought;
(b) What was the amount of reimbursement sought;
(c) What was the amount of reimbursement paid; and
(d) What amount was recovered from bail money;
(5) Did the defendant have more than one (1) counsel; if so, the number of counsel.
