Upon a unitization order becoming effective the commission shall file for recording in the office of the register of deeds in the county or counties in which the lands are located a certificate in such form as the commission may prescribe, which certificate must include the following information: (1) Date of order and date approved by required percentage of owners;
(2) docket number;
(3) name of unit;
(4) producing formations unitized;
(5) the time unit operations shall commence;
(6) legal description of each tract of land in the unit;
(7) the allocation of the production of the unit among the owners.
When unit operations have terminated, the commission shall likewise file for recording a similar certificate showing such termination.
