1. The department shall prescribe forms for the registration and record of children cared for by any child-placing agency licensed under this chapter and for reports required by the department from the agencies.
2. If, for any reason, a child-placing agency ceases to exist, all records of registration and placement and all other records of any kind and character maintained by the child-placing agency shall be turned over to the department, to be preserved and maintained by the department as a permanent record.
