Florida Statutes Title XXXVII. Insurance § 626.744. Service representatives, managing general agents;  application for license

The application for a license as service representative or the application for a license as managing general agent shall show the applicant's name, residence address, name of employer, position or title, type of work to be performed by the applicant in this state, and any additional information which the department may reasonably require.
