The social worker section, marriage and family therapist section, or professional counselor section may not grant any certificate or license under this subchapter unless the applicant does all of the following:
(1) Submits an application for the certificate or license to the department on a form provided by the department.
(2) Pays the fee specified in s. 440.05(1).
