(a) The president or vice president, on behalf of the telephone cooperative, shall execute and acknowledge the approved articles of amendment. The cooperative's seal must be affixed to the articles of amendment and attested by its secretary.
(b) The articles of amendment must state:
(1) that the articles of amendment are executed under this chapter;
(2) the name of the telephone cooperative;
(3) the address of the cooperative's principal office; and
(4) the amendment to the articles of incorporation.
(c) The president or vice president executing the articles of amendment shall make and attach to the articles an affidavit stating that the cooperative complied with this subchapter with respect to the amendment set forth in the articles.
