The board shall issue a license under this chapter to a license holder whose license has been lost or destroyed on:
(1) submission of an affidavit stating:
(A) that the license has been lost or destroyed;
(B) that the person making the affidavit is the person to whom the license was issued; and
(C) any other information required by the board; and
(2) payment of a fee for issuance of a duplicate license.
