All scire facias issued on conditional judgments and decrees rendered by any of the courts of this state on forfeited bonds, recognizances, subpoenas, summons, and other like obligations may be substantially in the following form, to wit:
State of Tennessee, county of __________.
To the sheriff [or coroner, as the case may be] of the county:
You are hereby commanded in the name of the state to notify __________ and __________ that a conditional judgment was rendered against them in the circuit court [or whatever court it may be] of __________ county, for the sum of $___ and costs on a forfeited bond [or recognizance, or whatever obligation it is], and the same will be made final unless you appear at the next term of court [or whatever the time may be under the order of the court], and show cause to the contrary.
Signed by the clerk, etc., with proper endorsement as to date of, etc.
