(a) All written statements pursuant to section 302(a)(2), 1 and all applications, petitions, and certifications required under the provisions of this act shall be made subject to the penalties provided under 18 Pa.C.S. § 4904 (relating to unsworn falsification to authorities) and shall contain a notice to that effect.
(b) All such applications, petitions, statements and certifications shall be submitted to the county administrator in the county where the person was made subject to examination and treatment and such other county in the Commonwealth, if any, in which the person usually resides.
(c) Subsections (a) and (b) shall not apply to patients admitted pursuant to Article II 2 when no part of the patient's care is provided with public funds provided that the department may require facilities to report clinical and statistical information so long as the data does not identify individual patients.
(d) Repealed by 1980, Oct. 5, P.L. 693, No. 142, § 331, effective in 60 days.
