(A) The registrar of motor vehicles or a deputy registrar shall ask whether each of the following wishes to make a two-dollar voluntary contribution to the second chance trust fund established under section 2108.34 of the Revised Code:
(1) A person applying for or renewing a driver's license, motorcycle operator's endorsement, or duplicate;
(2) A person applying for or renewing an identification card or duplicate;
(3) A person applying for or renewing a commercial driver's license, restricted commercial driver's license, or duplicate.
(B) The registrar or deputy registrar also shall make available to the person informational material provided by the department of health on the importance of organ, tissue, and eye donation.
(C) All donations collected under this section during each month shall be forwarded by the registrar or deputy registrar not later than the fifth day of the immediately following month to the treasurer of state, who shall deposit them in the second chance trust fund.
