Any person who shall release information which might disclose the identity of the woman in connection with a certificate of fetal death or report of fetal death in violation of the provisions of this title shall be subject to a civil penalty not to exceed five thousand dollars for each such release.  Such penalty may be recovered in the same manner as the penalty provided in section twelve of this chapter.
