Whenever a health care provider or practitioner makes a recommendation regarding the necessity of long term care services or a referral for the receipt of long term care services to a patient, the patient or patient's designated representative shall be provided by the health care provider or practitioner the contact information for NY Connects: Choices for Long Term Care, established pursuant to subdivision eight of section two hundred three of the elder law, that corresponds to the patient's county of residence or prospective county of residence based on the preference of the patient.
