1. The commissioner of the division of criminal justice services in cooperation with the office shall develop and implement a standardized procedure to be used by police officers, county sheriffs' departments and state police officers whereby victims of crime are notified about the rights of crime victims and the existence of programs designed to assist crime victims.
2. In establishing a victims assistance notification procedure, consideration shall be given to (a) developing a uniform method of informing victims of crime of their rights and services available, (b) including notification as part of a routine task performed in the course of law enforcement duties, and (c) documenting a victim's receipt of such notice.
3. All state or municipal printed forms for a police primary investigation report shall include a space to indicate that the victim did or did not receive information on victim's rights, office of victim services assistance and relevant local assistance pursuant to subdivision one of section six hundred twenty-five-a of this article.
4. In addition, the commissioner of the division of criminal justice services, in cooperation with the office, shall develop and implement a standardized procedure to be used by police officers, county sheriffs' departments and state police officers whereby family members and dependents who may be eligible to receive awards under this article are notified about the existence of such awards and the existence of any other relevant services or programs.
