Written application for any permit required under NRS 692B.040 in connection with the formation of a domestic mutual insurer by the securing of qualifying applications for insurance as provided in NRS 692B.200 shall be filed with the Commissioner. The application shall contain such information and be accompanied by such of the documents and the fees required under NRS 692B.070 as may be applicable, and in addition shall be accompanied by:
1. A copy of each policy for which applications are proposed to be solicited;
2. A copy of the proposed insurance application form, consistent with the requirements of NRS 692B.220 (qualifying applications for insurance); and
3. A schedule of the premiums or premium rates proposed to be charged in connection with such insurance applications.
