Upon payment of the renewal fee, submission of evidence of completion of the required number of hours of continuing education and submission of all information required to complete the renewal, the Executive Director shall execute and issue a renewal card for the license to the licensee, certifying that the license has been renewed for a 24-month period beginning March 1 of each even-numbered year. The renewal card must indicate the address of the place of the licensee's practice for which the card is issued and be displayed prominently at that location. The renewal card must be signed by the Executive Director.
