Application for a license to conduct such an assembly shall be made in writing to the city clerk at least 60 days prior to the time indicated for the commencement of the planned activity and shall be accompanied by a nonrefundable application fee in the amount established by the city council. The application shall include:
1. The name and address of the applicant or applicants.
2. The legal description of the place where the proposed assembly is to be held.
3. The date or dates of the assembly.
4. The estimated attendance at the assembly.
5. The nature or purpose of the assembly.
6. Such other information as the city council determines is necessary.
