When a guardian has taken the official oath and filed a bond as provided in this chapter, the court shall order letters of guardianship to issue to the guardian. Letters of guardianship may be in the following form:

On __________ (month) __________ (day), __________ (year), the __________ Judicial District Court, __________ County, State of Nevada, appointed __________ (name of guardian) __________ (guardian of the person or estate or person and estate) for __________ (name of protected minor), a minor, that the named guardian has qualified and has the authority and shall perform the duties of __________ (guardian of the person or estate or person and estate) for the named protected minor as provided by law.
In Testimony Whereof, I have hereunto subscribed my name and affixed the seal of the court at my office on__________ (month) __________ (day), __________ (year).
