Nevada Revised Statutes Title 13. Guardianships; Conservatorships; Trusts § 159.075. Letters of guardianship

When a guardian has taken the official oath and filed a bond as provided in this chapter, the court shall order letters of guardianship to issue to the guardian. Letters of guardianship may be in the following form:

On __________ (month) __________(day) __________(year) the __________ Judicial District Court, __________County, State of Nevada, appointed __________(name of guardian) __________(guardian of the person or estate or person and estate or special guardian) for __________(name of protected person), an adult, that the named guardian has qualified and has the authority and shall perform the duties of __________(guardian of the person or estate or person and estate or special guardian) for the named protected person as provided by law.

In Testimony Whereof, I have hereunto subscribed my name and affixed the seal of the court at my office on __________ (month) __________(day) __________(year).
