A person who is registered as a governmental affairs agent and who, on or after the effective date of P.L.1991, c. 243 (C.52:13C-18 et al.) or on or after P.L.2004, c. 27 for the purpose of influencing governmental processes, serves or shall serve as a member of any independent State authority, county improvement authority or municipal utilities authority, or as a member from New Jersey on an inter- state or bi-state authority, or as a member of any board or commission established by statute or resolution or by executive order of the Governor or by the Legislature or by any agency, department or other instrumentality of the State shall disclose such service, including the name of the authority, board or commission and the date upon which his term as a member thereof expires, in the annual report required to be made under section 2 of P.L.1981, c. 150 (C.52:13C-22.1).
