New Jersey Statutes Title 26. Health and Vital Statistics 26 § 2-127

A community based high blood pressure control program shall provide:

a. Hypertension screening;

b. Treatment referral;

c. Counseling forpersons with high blood pressure as to proper diet, weight control, and appropriate care and treatment of the condition; and

d. Patient follow-up and evaluation of treatment methods.
