The certificate of acknowledgment by an attorney in fact must be substantially in the following form:

On this ___ day of __________, in the year __________ before me (here insert the name and quality of the officer), personally appeared __________, known to me (or proved to me on the oath of __________) to be the person who is described in and whose name is subscribed to the within instrument as the attorney in fact of __________ and acknowledged to me that that person subscribed the name of __________ thereto as principal and that person's own name as attorney in fact.
