North Dakota Century Code Title 47. Property § 47-19-27. General certificate of acknowledgment

A certificate of acknowledgment, unless otherwise provided in this chapter, must be in substantially the following form:

STATE OF NORTH DAKOTA

County of __________________

)

On this ___ day of __________, in the year __________ before me personally appeared __________, known to me (or proved to me on oath of __________) to be the person who is described in and who executed the within instrument, and acknowledged to me that that person (or they) executed the same.
