Mississippi Code Title 73. Professions and Vocations § 73-31-11

(a) The name, age and residence of each applicant;

(b) The date of the application;

(c) The place of professional practice of the applicant;

(d) A summary of the educational and other qualifications of the applicant;

(e) Whether or not an examination was required;

(f) Whether or not a license was granted;

(g) The date of the action of the board;

(h) Any other information as may be deemed necessary or advisable by the board in aid of the above requirements.
