The ballot at said special removal election shall be in the form usually prescribed for special elections, and shall be headed “Official Ballot.” Each ballot shall contain the following instructions to the voters: “Mark a cross (X) or check mark (✓) in the square at the right of ‘yes’ or ‘no’.” Immediately thereunder shall appear the question: “Shall __________ (naming the officer) be removed from the office of __________?” Immediately under such question shall be printed the words “yes” and “no”, and immediately following each of said words shall be placed a square in which the voter may indicate his preference.
