Any such person shall make application for the special license plates on a form prescribed by the director of revenue, and shall submit the application and proof of his disability in the form of a statement from the United States Veterans Administration verifying the person was awarded an automobile by the United States Federal Government because of a service incurred disability and/or the person is receiving compensation for total service connected disability.
