Sec. 14. An instrument in substantially the following form satisfies the requirements of section 13(2)(b) and 13(8): 1
“TRANSFER UNDER THE MICHIGAN UNIFORM TRANSFERS TO MINORS ACTI, ____________ (name of transferor or name and representative capacity, if a fiduciary) transfer to ____________ (name of custodian), as custodian for ____________ (name of minor) under the Michigan uniform transfers to minors act, the following:
(insert a description of the custodial property sufficient to identify it).

________________ (name of custodian) acknowledges receipt of the property described above as custodian for the minor named above under the Michigan uniform transfers to minors act.
