Kentucky Revised Statutes Title IV. Judicial Branch § 27A.390.Court disposition level relating to offenses charged and offenses disposed of

(1) The court which disposed of the offense;

(2) The name of the presiding judge;

(3) Date upon which the trial began;

(4) The final pleading, as to each charged offense and the level thereof;

(5) Listing of charged offenses disposed of by the court together with:

(6) Listing of offenses for which the defendant was convicted by KRS number, name and level;

(7) Number of convicted offenses; and

(8) Date of conviction.
