Sec. 0.5. Upon the filing of a petition to establish paternity, the clerk of the court shall prepare a notice of the filing on a form prescribed and furnished by the Indiana department of health. The notice must include the following:
(1) The name of the child.
(2) The name of the mother of the child.
(3) The name and address of the man alleged or alleging to be the father of the child.
(4) The name of the petitioner.
(5) The date the petition was filed.
(6) The name of the court and cause number.
