Indiana Code Title 12. Human Services § 12-30-7-4

Sec. 4. (a) A county described in section 1(a) of this chapter shall consolidate all real property, buildings, equipment, supplies, employees, insurance, and equity of the county home and the hospital and operate both as one (1) entity to be known as the “__________ Health Center”.

(b) The health center must include at least the following two (2) divisions:
