§ 35-15. Statement of termination. When a limited liability company has been wound up, a statement of termination shall be executed in duplicate in the manner prescribed in Section 5-45 and shall set forth all of the following:
(1) The name of the limited liability company;
(2) A post office address to which may be mailed a copy of any process against the company that may be served upon the Secretary of State; and
(3) A statement that the limited liability company has been terminated.
