§ 25.2. Office of State Guardian birth record request.
(a) For purposes of this Section, an individual's status as a person under guardianship with the Office of State Guardian may be verified with a copy of the court order placing the individual under the guardianship of the Office of State Guardian.
(b) The applicable fees under Section 17 for a new certificate of birth and under Section 25 for a search for a birth record or certified copy of a birth record shall be waived for requests made by the Office of State Guardian to the Office of the State Registrar of Vital Records in Springfield for an individual under guardianship of the Office of State Guardian, whose status is verified under subsection (a) of this Section.
(c) The State Registrar of Vital Records shall establish standards and procedures consistent with this Section for waiver of the applicable fees.
(d) An individual under guardianship shall be provided no more than 4 birth records annually under this Section.
