§ 50. Application for original license. Applications for original licenses shall be made to the Department on forms prescribed by the Department and accompanied by the required fee, which is not refundable. All applications shall contain information that, in the judgment of the Department, will enable the Department to approve or disapprove of the qualifications of the applicant for a license to practice as a professional music therapist. If an applicant fails to obtain a license under this Act within 3 years after filing the application, the application shall be denied. The applicant may make a new application, which shall be accompanied by the required nonrefundable fee. The applicant shall be required to meet the qualifications required for licensure at the time of reapplication.
