§ 9-12.1. (a) On the reverse side of each ballot contained in Section 9-12, except the ballot under Format 6, shall be printed the following:
OFFICIAL BALLOT․ County, IllinoisSchool District No. ․, ․ County, IllinoisElection Tuesday, (insert date) (facsimile signature of the election authority)(b) If 6-year terms have been adopted under Section 9-5, or if a ballot is to be used to elect a member or members of a board of school directors or board of education at the consolidated election held in April of 1999 or April of 2001 to a full term that is less than a 4-year term, appropriate adjustments should be made to each ballot in Section 9-12. In the case of any unexpired term each ballot format must indicate whether it is a 4-year or a 2-year unexpired term.
