Any person who shall desire to begin the practice of optometry in the state of Idaho shall file an application in his own handwriting on forms furnished by the board for an examination, which application shall state among other things, his correct name, age, place of residence, the name and location of the school or schools attended by the applicant, and date of such attendance, and whether the applicant is graduated from such school, and the place and with whom he has served as an intern and the exact length of time and nature of such internship. Such applications must be signed by the applicant under oath.
