(a) Within thirty days of a legal name change, a program participant shall provide the program with a certified copy of a judgment, order, or any other documentation the program director deems to be sufficient evidence of the name change.
(b) Within ten days of a change in actual address or telephone number, a program participant shall notify the program of the change.
