Within 24 hours after the cremation or natural organic reduction is completed a report indicating the name of the individual, the individual's address while alive, the date and cause of death, the names of the individuals signing the permit, the date of the completion of the cremation or natural organic reduction, and the disposal of the final remains must be forwarded by the person in charge of the crematory or natural organic reduction facility to the central Office of Vital Statistics.
