The form of the petition for proposing a consolidation shall be as follows: WARNING: ALL SIGNATURES SHALL BE IN INK OR INDELIBLE PENCIL. We, the electors of the town, city or borough or unit of local government of (Here insert the name of the town, city, borough or unit of local government), hereby present this petition under the provisions of section 7-195 proposing a consolidation with the following-named town, city, borough or unit of local government ․․․․, and we certify that we are electors of the town, city, borough or unit of local government of ․․․․ residing at the addresses set opposite our names and that we have not signed this petition more than once.
