(a) The Department of Public Health shall prescribe forms for an acknowledgment of parentage. Such forms shall include the minimum requirements specified by the Secretary of the United States Department of Health and Human Services, contained in 45 CFR 303.5, as amended from time to time, and shall be in compliance with the provisions of public act 21-15. 1 Any acknowledgment or rescission executed in accordance with this subsection shall be filed in the parentage registry established and maintained by the Department of Public Health under section 19a-42a.
(b) A valid acknowledgment of parentage is not affected by a later modification of the form under subsection (a) of this section.
